If you are a new patient to Sanders Therapy Group for Physical Therapy, Hand Therapy, or Pediatric Occupational therapy or Speech services, we have downloadable forms below that you can print and fill out before visiting one of our clinics. If you call our reception staff, they can send you a link for some of the online registration as well.

When initially contacting our reception staff, they will likely direct you to electronic registration forms. If that is not an option, please download and print new patient registration forms and bring them with you to your first visit.
